New Patient Information
What to Expect
Let’s get acquainted! Your initial exam will be a thorough, comprehensive evaluation of your dental health. Our practice goal is to encourage good hygiene habits, increase patient awareness of products and home care, treat any and all current conditions, and identify vulnerability before it affects your healthy smile. We evaluate every aspect contributing to your dental health, using a variety of evaluation methods:
- Periodontal probing measurements, to assess the gumline for any potential detachment or pockets. In cases of neglect, these pockets and detachment can lead to bacterial infections or bone loss.
- Radiography and SOPRO fluorescence imaging, to evaluate enamel for decay and demineralization.
- Physical evaluation of the jaw joint and bite, to determine if TMJD, clenching, or grinding contribute to any physical or dental ailments.
- Oral Cancer Screening
- In many cases, diagnostic photos or models will be incorporated to better illustrate the occlusion and alignment that is unique to your specific bite.
The below patient forms can be printed and completed before your first visit to expedite your check-in process:
Insurance, Payments, & Financial Arrangements
Our office accepts all major credit cards except American Express.
In order to provide the highest quality care in the most accessible manner, we have developed a full range of convenient payment options for you to choose from. Prior to the commencement of treatment, financial arrangements will be discussed with you. Our front desk is also ready and willing to help you with any questions you may have about your insurance benefits.
Payment of fees for routine services, such as examinations, teeth cleaning, X-rays and emergency visits, are expected at the time that services are provided. Insurance claims are prepared assigning benefits to the patient so that your insurance carrier reimburses you directly. We accept cash, Visa, MasterCard, Discover, and personal checks, and we also offer interest-free payment plans both in office and through Care Credit.
Do not hesitate to ask about the anticipated investment in our dental health. You have the right to know and understand our fees for care. We strive to treat our patients as we would want to be treated and we want you to feel that you have been treated fairly. We will gladly give you a written treatment plan including an estimate for any necessary care upon request. For those with dental insurance benefits, a pre-determination of benefits can be obtained to calculate your out of pocket investment for recommended treatment. We ask for an initial payment for more involved and extended treatment, regardless of insurance coverage or pre-determination of payment by your insurance company. Our dental team thanks you for your understanding and cooperation.
What is the difference between in-network and out-of-network providers?
An in-network provider is one contracted with the health insurance company to provide services to plan members for specific pre-negotiated rates. An out-of-network provider is one not contracted with the health insurance plan.
Typically, if you visit a dentist or other provider within the network, the amount you will be responsible for paying may be different than if you go to an out-of-network provider.
As a courtesy, our office will submit all dental claims on your behalf. We will also include any necessary documentation and/or attachments for prompt reimbursement.
Please refer to your Insurance Benefit’s Handbook for further details.
We do something very simple at CareCredit: we help people get the care they want for themselves and their families.
And, for us, that’s very important. Together, with more than 175,000 health care providers, we’re helping people access care.
Thousands of times each day, people are using their CareCredit health, wellness and beauty credit card to have important dental work done, be fitted with new hearing aids, have vision or cosmetic procedures, and get care for a beloved family pet.
We help make it easy for people to get the care they want and need, right when they need it, without having to delay treatment or appointments.
For almost 30 years, CareCredit has been providing a valuable financing option for treatments and procedures that typically are not covered by insurance, or for times when insurance doesn’t cover the full amount. CareCredit is also used by cardholders to pay for deductibles and co-payments.
We think people like having financial options because that gives them the freedom to make decisions about treatment options. So they can do what’s best for their situation and their family.
That’s why CareCredit is here. We help make care possible … today.
CareCredit, from Synchrony Financial, is one of the largest and most popular health, wellness and beauty credit cards in the nation, serving millions of families each year.
